Exercise and Education Programs Focused on Prevention and Chronic Disease Management
Exercise and education play a vital role in prevention and managing chronic health conditions. Yet, for many, beginning an exercise routine can be daunting given physical limitations, safety concerns, lack of information about the disease process and a variety of other factors.
Cooper® Tracks provides participants guidance and support to help them begin exercising, increase confidence and develop habits to live healthier and best manage their chronic conditions. Cooper Tracks comprehensive health education programs give participants the opportunity to learn how lifestyle modifications can positively affect their well-being and specific health conditions. A physician referral is required prior to participation in the Cardiovascular, Cancer, and Arthritis Tracks.
Cooper Tracks is designed for:
- Sedentary individuals who want to begin an exercise program
- Individuals who have been diagnosed with chronic disease or risk factors for disease
- Individuals transitioning from a completed rehabilitative program to regular physical activity
- Individuals following a preventive approach via exercise and lifestyle modification
Cooper Tracks is an eight-week program featuring:
- 16 small group exercise/education sessions (two/week)
- Individualized exercise in a group setting, led by a certified professional fitness trainer
- Comprehensive health education on topics related to chronic disease, nutrition, healthy lifestyle and stress management
- Pre-and post-program health assessments
- Social support and accountability
- Exercise tracking tools
Tracks Available at Shannon Health Club
- Diagnosed with stable cardiovascular disease or cardiovascular risk factors • Completed or do not qualify for cardiac rehabilitation • Peripheral arterial disease
Tuesday/Thursday: 9-10 am

Monday/Wednesday: 11:00 am – Noon; 1:00 to 2:00 pm; 3:00 to 4:00 pm
Tuesday/Thursday: 1:00 – 2:00 pm

- Diagnosed with cancer and undergoing treatment • Recovering from cancer at any stage
Tuesday/Thursday: 11:00 am – Noon

- Diagnosed with Pre-diabetes • Diagnosed with Type 1 or Type 2 diabetes
Monday/Wednesday: 9:30-10:30 am; Tuesday/Thursday: 3:15 to 4:15 pm
